Terms of Service

1. Acceptance

By creating an account or using IntakeCompass ("the Service"), you agree to these Terms. If you are using the Service on behalf of a practice or organization, you represent that you have authority to bind it.

2. What the Service is (and isn't)

IntakeCompass provides structured, non-diagnostic intake assessments for licensed clinicians. It organizes a client's self-reported responses into a summary. It is not a medical device, does not provide a diagnosis or treatment, and is not a substitute for your professional clinical judgment. It is also not an emergency service.

3. Eligibility

The Service is intended for licensed mental-health and healthcare professionals acting within their scope of practice. You are responsible for ensuring your use complies with your licensure, applicable law, and professional ethics.

4. Accounts and security

You must provide accurate information, keep your credentials secure, and enable the required two-factor authentication. You are responsible for activity under your account.

5. Clients, consent, and PHI

You decide which clients receive an assessment link and are responsible for obtaining any consent required by law and your professional standards. You are the responsible party for your clients' protected health information; we act as your Business Associate and process that information on your behalf under our Business Associate Addendum and Privacy Policy.

You agree not to misuse the Service — including attempting to breach security, access another practice's data, use the Service for unlawful purposes, or send assessments to people without an appropriate clinical relationship and consent.

8. Availability and changes

We work to keep the Service reliable but do not guarantee uninterrupted availability. We may modify or improve features over time, and will give reasonable notice of material changes where practical.

9. Disclaimers and limitation of liability

The Service is provided "as is." To the fullest extent permitted by law, we disclaim implied warranties and are not liable for indirect or consequential damages. Nothing in the Service replaces your independent clinical judgment or your obligations to your clients. [Specific liability caps and warranty language to be finalized with counsel.]

10. Termination

You may stop using the Service at any time and request deletion of your organization's data. We may suspend or terminate accounts that violate these Terms or applicable law.
